Fighting health inequality

Eric Brunner favors political efforts to reduce health inequality within rich countries. I would reduce poor health rather than health inequality. And I think that it is more urgent to do so in poor countries than in rich ones. As the deep cause of bad health among the poor is poverty we should perhaps make an overall effort to fight poverty. Given that resources are limited, what is the moral basis for spending them to fight health inequality within rich countries?
